CHILD DROP OFF REGISTRATION FORM

    Paint N Play RVA Policies, Release of Liability, and Assumption of Risk Agreement

    1) Behavioral Problems. I understand that if my child is continually disruptive, is physically or verbally abusive toward the instructor or other students, or exhibits other ongoing behavioral problems during the class, that I will be notified by phone call to come pick-up my child immediately and will not receive any refund of tuition.
    2.) I knowingly and freely assume all such risks, both known and unknown, and assume full responsibility for my child’s participation in Paint N Play RVA LLC activities. I acknowledge that my child does not have any allergies or health conditions that would prevent him or her from participating in the Paint N Play RVA LLC activities and that participant has adequate health insurance to cover the costs of treatment in the event of any injury.
    3.) I, for myself and/or my child(children) and on behalf of my heirs, assigns, personal representatives and next of kin, herby release and hold harmless Paint N Play RVA LLC and all persons acting on their behalf, and their agents or employees, and if applicable, owners, lessors of the premises used to conduct the activities (“releases”) with respect to any and all injury, disability, death, or loss to person or property.
    4.) Cancellation/Refund Policy: A studio credit in the amount of  your registration fee may be provided if you cancel in writing to [email protected] prior to 48 hours before your event. Due to the high demand of our Kids Programs, limited studio space, and art and craft material preparation, no refunds, exchanges, or credits will be given inside 48 hours. Please plan accordingly.

    By typing my name and initialing in the box below, I am authorizing the participation of my child and have accepted the terms.
